The causes can be:

- Bad/worn throw out bearing

​

- Bad/worn Pilot bearing

 

- Bad/worn clutch disc

​

- Bad/worn clutch pressure plate

​

If you are considering fixing this, change all 4 of the above, especially the 2 bearings, as otherwise you might still get the same problem again.
